Quick Answer: What Are Nightmare Doors?
Fourteen optional challenge portals scattered across the missions. Each one is a self-contained combat-and-traversal trial rated Bronze, Silver or Adamantium, and finishing one unlocks an audio log of one of Logan's memories β the fragments of his past that the story slowly assembles. The first two unlock naturally through the story; discovering the second one (The One That Got Away, Canada) is what switches on mission select.
Why They Matter More Than They Look
Three systems hang off the doors. Adaptation levels: doors (plus Whisky Bottles) are the only Adaptation XP β level 10 pays the Secondary Mutations trophy and the finders that make cleanup painless. The hidden trial: bronze-or-better in all 14 spawns the 15th trial (Best There Is) post-story, which leads to the hidden epilogue and the Repressed Memories trophy. And the story: Push Square's review notes the memory logs carry Logan's backstory through the campaign's first half β skipping the doors means skipping the plot.

The Ratings and How Lenient They Are
Trials score Bronze through Adamantium, and launch-week coverage is unanimous that the requirements are forgiving β PowerPyx reports first-try Adamantium in every trial, and notes you can run them on The Man difficulty where nothing can kill you. You never need Adamantium anywhere except the single trial for Adamantium Dreams; Bronze everywhere else clears every trophy requirement.
Where the Doors Hide
Doors sit along and just off the mission path β the same shader trick (Settings > Visuals) that outlines crates outlines interactables, and the per-mission checklist in the Nightmare Cabin (interact with the bed) shows exactly which missions still owe you a door.
